Bummer. As you can see by any number of posts at this site, Murphy and
his Law is alive and well in most of our shops. It's not a question of if he
will strike, only when.

I don't know if this would solve your problem in the future or not, but look it over. On my bender I made four retainers from scrap pieces of steel and put two on each side. They keep the sandwich pretty well in place.   I bend the waist first.






This may not have anything to do with what caused your trouble, but someone might find it useful.
