Which acoustic guitars do you feel have the best sound? Odds are, that same maker has a 12 string that sounds really nice too. I would head out to the nearest Guitar Center or other music store and play away. I personally like Martin's, but this is a very subjective thing.

Factory made 12 string I like Taylor, Alverez Yairi and early 70's model Yamaha (the SG270). As far as size of guitar, most are dreadnaught bodies but Taylors 555 and 655 are both very good 12 strings. I have built my SJ in a 12 string variant and I like it a lot. I built one OM 12 string it was pretty good.

Yes. Like Michael pointed out, most makers use the dread shape. I have seen some jumbo sized 12 strings too.

UK guitar builder Dave King builds a 12 string parlour guitar. Don't be limited by the major manufacturers body shapes, they are building for a mass market, if it's for yourself just adapt your favourite body.

Colin, that is a great looking guitar! I wonder how the smaller body affects the sound...It makes sense though, there are plenty of multicourse instruments from the South American continent that have small bodies like this Columbian Tiple:
