Post subject:  insert issues

I just noticed this weekend during a jam session that my dobro would not stay in tune. This has never really been a problem with this instrument before but upon investigation I notice that string heights are off. This was new and the string that was low was the culprit concerning the intonation... going flat.

Question:

As I am new to resonator insert issues (currently building my first one), I lack knowledge with insert lifespan. My current insert is all maple and was installed about 15 years ago when I had the original cone and nut replaced. For information, this is a roundneck 60 and I had it set up for slide. Do inserts wear out? I thought maybe that a small split may have developed below that strings slot. I have yet to disassemble so nothing concrete regarding my hypothesis...?

Any suggestions, advice, welcomed

Maybe the inserts weren't 'inserted' all the way I usually run a file in the slot and some builders actually run an end mill through them to get them square. I install my inserts with a vise too, to make sure they are bottomed out fully.
